Key Updates: Indian & ESC guidelines maintain 140/90 threshold vs ACC/AHA 130/80. Use 140/90 for general population, but treat CAD/stroke/HF patients at 130/80. Combination therapy preferred for most patients requiring treatment. ACEI/ARB + CCB or diuretic are optimal first-line combinations. Target <130/80 in <60 years, allow 130-140 in elderly if needed.
